PLEASE FOLLOW THIS POLICY SO WE CAN BETTER SERVE YOU

ALLOW 5 DAYS FOR REFERRALS TO BE DONE. PLEASE MAKE YOUR APPOINTMENTS WITH THE SPECIALIST BEFORE CALLING FOR YOUR REFERRAL. PLEASE PICK UP REFERRALS WITHIN THE WEEK YOU CALLED FOR IT. PATIENT MUST BE REFERRED TO SPECIALIST BY MILLBURN PRIMARY CARE.

ALL PATIENTS ARE EXPECTED TO PICK UP THEIR REFERRALS. REFERRALS CANNOT BE FAXED. IF NOT, PLEASE PROVIDE A SELF-ADDRESSED STAMPED ENVELOPE. YOU MAY GIVE A FRIEND A NOTE ALLOWING THEM TO PICK UP THE REFERRAL FOR YOU.

REFERRALS DO EXPIRE, PLEASE PICK THEM UP PROMPTLY AND USE THEM AS SOON AS POSSIBLE. SPECIALISTS CANNOT SEE YOU WITHOUT A REFERRAL. DO NOT SEE A SPECIALIST WITHOUT A REFERRAL. WE CANNOT BACK DATE REFERRRALS. A SPECIALIST WILL BILL YOU IF YOU SEE HIM WITHOUT A REFERRAL.

YOU MUST BE ONE OF OUR CURRENT PATIENTS AND HAVE HAD A VISIT WITHIN THE YEAR FOR US TO DO THE REFERRAL.

IF OUR DOCTORS BELIEVE YOU HAVE A MEDICAL EMERGENCY REQUIRING AN IMMEDIATE VISIT WITH A SPECIALIST, WE WILL DO THAT REFERRAL FOR YOU RIGHT AWAY.
