REFERRAL POLICY:


Please follow this policy so we can better serve you!

To request referrals, leave the following information:

        Full name
        Day time phone
        Name of medical facility you wish to see (include the doctor's address and phone number)
        The date of your appointment
        The reason for your visit

 

Please allow 3 business days for referrals to be done. Please make your appointments with the specialist before calling for your referral. Please pick up referrals within the week you called for it. Patients must be referred to their specialists by Millburn Primary Care.

Celeste will always inform you when your referral is ready to be picked up. You should come in a minimum of once per year to obtain a referral.

WE DO NOT FAX REFERRALS. Referrals may be accessed in two ways:

In Person: please stop by the office any time between 9am and 4pm weekdays or 8am-Noon on Saturdays. When possible, please call in advance to make sure it has been completed and is ready for pickup. With your prior approval, we will always release your referrals to a trusted family member or friend.

Online: Horizon Blue Cross Blue Shield and Oxford members may retrieve and print referrals. You must be registered with your plan's site to use these features. Aetna online retrieval is not available at this time.

Due to the Privacy Rule under the Health Insurance Portability and Accountability Act, referrals may not be left outside our door or delivered by email. Thank you for your cooperation.

Referrals do expire, please pick them up promptly and use them as soon as possible. Specialists cannot see you without a referral. Do not attempt to see a specialist without a referral; under the limitations of your plan, you may be billed for any office visits not authorized by your chosen Primary Care Physician.

You must be one of our current patients and have had a visit within the year for us to do the referral.

If our doctors believe you have a medical emergency requiring an immediate visit with a specialist, we will do that referral for you right away. Please do not abuse this policy.
